This Apollo 16 insurance cover is from the personal collection of Lunar Module Pilot Charlie Duke, and signed by John Young, Ken Mattingly and Duke shortly before their launch.
This was one of a small quantity of covers that were left behind as a form of "insurance" in the event they failed to return from the Moon.
